Family and Medical Leave Act (FMLA) and Short-Term Disability can be used when you need to take time off work for medical reasons or pregnancy. These are two options for taking leave, and each has a different purpose.
All FMLA forms must be sent to Gwinnett Ob/Gyn. The patient is responsible for contacting their employer to obtain the correct form. These forms can be faxed: 770 985-2683, or brought to one of your appointments.
It is important to speak with your employer to discuss your benefits before you turn in your Short-Term Disability and/or FMLA paperwork, so you understand fully. It is important to let Gwinnett Ob/Gyn know how long you plan to be out of work or how long your support person is going to be out so we can ensure your forms are filled out accurately. FMLA and/or Short-Term Disability forms can take up to 7 business days to complete.
