How Does Each Type of Ultrasound Work?

Every ultrasound starts the same way. A handheld device called a transducer sends high-frequency sound waves into the body. Those sound waves bounce off internal structures, including the baby, placenta and amniotic fluid. The transducer receives the returning echoes and converts them into images on a screen.

The difference between 2D, 3D and 4D imaging comes down to how the machine processes those returning sound waves.

  • 2D ultrasound captures a single plane of sound waves at a time. The result is a flat, grayscale image that shows a cross-sectional slice of whatever the transducer is pointed at. Trained sonographers and providers read these images to evaluate internal organs, skeletal structure, fetal anatomy, placenta position and amniotic fluid levels. A 2D scan is the standard format for all diagnostic pregnancy ultrasound appointments.
  • 3D ultrasound captures multiple 2D images from many different angles in rapid succession. Software compiles those images into a single still picture that shows the surface of the baby’s body in three dimensions, giving parents a lifelike view of the baby’s appearance.
  • 4D ultrasound uses the same multi-angle capture as 3D imaging, but it processes frames continuously to create a live video feed. The fourth dimension is time. Parents watch their baby move in real time on the screen.

The baby still has enough room in the uterus to move freely, which produces better 4D video. Earlier than 24 weeks, surface detail may be limited. Later than 32 weeks, tighter space in the uterus can restrict the baby’s movement and make it harder for the transducer to capture clear angles.

Are 3D and 4D Ultrasounds Safe?

Yes. A 3D ultrasound and a 4D ultrasound use the same sound wave technology as a standard 2D scan. No radiation is involved in any type of ultrasound imaging. The American College of Obstetricians and Gynecologists (ACOG) states that ultrasound is safe when performed by a qualified provider for a medical indication or when ordered by a healthcare provider.

ACOG and the FDA advise against non-medical “keepsake” ultrasounds performed at commercial boutiques that are not supervised by a licensed healthcare provider. The concern is not with the sound wave technology itself but with unregulated session lengths, untrained operators and lack of clinical oversight. A 3D ultrasound or 4D ultrasound performed at your OBGYN’s office is conducted under physician supervision with properly calibrated equipment and appropriate session duration.

Insurance typically covers medically indicated ultrasound appointments, including the dating scan, anatomy scan and growth monitoring scans. Elective 3D ultrasound and 4D ultrasound sessions may not be covered by insurance. Your provider’s office can help clarify coverage before your appointment.

How Do You Get the Best 3D and 4D Ultrasound Images?

A few factors influence 3D ultrasound and 4D ultrasound image quality. Knowing what to expect can help you prepare for your appointment.

  • Schedule between 24 and 32 weeks. This is the optimal window for facial detail and fetal movement, as outlined in the timeline above. Many parents find that 26-30 weeks produces the clearest results.
  • Stay well hydrated. Drinking plenty of water in the days leading up to your appointment can improve amniotic fluid clarity. Clear fluid creates a better acoustic window between the transducer and the baby’s skin surface.
  • The baby’s position matters. If the baby is facing your spine or the placenta sits along the front wall of the uterus (anterior placenta), facial images may be limited during that session. Your sonographer may ask you to walk around or shift positions to encourage the baby to move into a better angle.
  • Patience helps. Some babies cooperate quickly, and others take more time. Your sonographer works with the baby’s natural movements to capture the best possible images and video.
